Harper County Community Hospital and SSM Health St. Anthony Hospital entered into a management agreement on October 1, 2019.  The management agreement helps Harper County Community Hospital with a wide range of management support services that will aid the facility as they continue to provide the highest quality of health care to the community.

HCCH is happy to offer our patients access to MyChart offers access to your labs and medical records  via your computer or mobile device.

To initially set up MyChart, an activation code from your electronic health record is necessary.  Please call the Laverne Family Health Clinic, Buffalo Family Health Clinic, or HCCH and they can assist you.
